Self Sufficiency Case Management
Self Sufficiency Case Management is provided to help those who need a plan for achieving consistent financial sustainability for their households. Financial assistance may be provided as a client progresses toward their goal of self-suffiency and meets the requirements of a carefully monitored case plan. Individuals are required to document all income and expenses for the household, participate in budget counseling, and other activities as required to complete their case plan.
The following items are required to verify eligibility:
- Verification of Social Security Numbers (cards) for all members of the household
- Verification of all income coming into the household (all members age 18 and older) for the past 30 days
- Current statements for services requested (utility, rent, and/or mortgage)
- Additional items as requested
